Wiley Creek Memory Care is the dedicated cognitive care building of ISF's Sweet Home, Oregon campus, a purpose-built 27,000 square foot memory care facility operating adjacent to the independent and assisted living community.

Wiley Creek Memory Care is a purpose-built cognitive care facility completed to address a documented gap in dementia care across the Sweet Home region.
Before construction of Wiley Creek Memory Care, families in Sweet Home seeking specialized memory care for loved ones had to travel as far as Albany, Corvallis, or Salem. The Sweet Home city government identified memory care as a top community priority, and the dedicated building was developed specifically to fill that gap.
ISF acquired Wiley Creek Memory Care in August 2025 as part of a three-community portfolio transaction financed through $79.32 million in tax exempt bridge financing. The acquisition added a purpose-built dementia care environment to ISF's growing operating footprint and expanded the Foundation's capacity for cognitive care research and resident participation programs.

Wiley Creek Memory Care operates as a research and innovation node in ISF's broader cognitive care network, with infrastructure connecting to Stanford, NIH, and Cedars-Sinai partnerships.
ISF's research partnerships create a natural pipeline for memory care communities to participate in early stage Alzheimer's detection work, AI driven posture and gait analysis, sleep quality monitoring, and the APOE genetic screening program ISF operates across the portfolio.
Every ISF memory care environment operates with the same clinical infrastructure and family engagement tools deployed across the full network, integrated into the ISAI data platform from the operating floor up. AI driven fall prevention and continuous monitoring provide care teams with early intervention signals before conditions progress.
Adjacent to Wiley Creek Senior Living on the same 42-acre campus, the memory care building enables seamless transitions for residents whose care needs evolve, with families navigating one community and one care relationship across the full continuum.
