Wiley Creek Senior Living anchors the independent and assisted living side of a multi-building campus, originally opened in 1998 and continuously operated as a full continuum senior living community on Sweet Home's eastern edge.
The Wiley Creek campus opened in January 1998, originally developed by Mid-Valley Medical Group and later operated by Samaritan Health Services. The community has served the Sweet Home market for over 25 years as one of the few full continuum senior living properties between Albany and the Cascade foothills, with independent living cottages and an assisted living lodge integrated across the 42-acre site.
ISF acquired Wiley Creek in August 2025 as part of a three-community portfolio transaction spanning Oregon and Montana. The deal was financed through $79.32 million in tax exempt financing. Wiley Creek's combined campus contributed 108 units to the 287 unit portfolio that established ISF's presence across the Pacific Northwest and Mountain West.

Wiley Creek Senior Living operates as a node in the ISF network, with the independent and assisted living buildings integrated alongside the adjacent memory care property under one connected operating model.
The acquisition thesis is not transformation. Wiley Creek has a 25 year operating history, an established team, and a stable resident base. ISF's role is to bring the connected operating model used across the broader portfolio to a campus that already runs well.
That means ISAI data platform integration, access to the same family engagement portal deployed across the network, and inclusion in the research and AgeTech partnership pipeline connecting ISF communities to Stanford, NIH, and Cedars-Sinai.
The campus structure, with independent living cottages, an assisted living lodge, and a dedicated memory care building all operating on the same 42 acres, allows residents to age in place through every transition without leaving the community.
